- Every request for records must be mailed to our Request Process Center: 1142 S. Diamond Bar Blvd PMB 310, Diamond Bar, CA 91765
- Each request for records must include an Initial Processing Fee for each type of record requested, including Medical, Billing, and X-ray records.
- Each treating facility must be served individually. Please do not request records from multiple treating locations in a single request.
